To receive your $50 STOKKE® Thank You Gift Card (American Express Gift Card), purchase a SLEEPI™ crib and STOKKE® mattress or SLEEPI™ System 1 and STOKKE® mattress and other STOKKE® product(s) of minimum $249.99 value (e.g. TRIPP TRAPP® , XPLORY® , Bedding Set) at participating retailers between 09/01/08 and 11/30/08, complete an official Claim Form (available online at http://www.stokkeusa.com/promo/sleepi0808/entryform.asp or at participating retailers) and submit along with a copy of your store receipt(s) with the qualifying STOKKE® product name circled as follows: Via Postal Mail: Send your official completed Claim Form and copies of your store receipt(s) to: ATTN: STOKKE SLEEPI PROMOTION Claims received without proper postage will not be processed.
